The resignation shall be forwarded to the Personnel Director with <br />a statement by the Police Chief as to the resigned employee's <br />service performance and other pertinent information concerning the <br />cause for resignation. <br />Failure of an employee to comply with this rule shall be entered <br />on the service record of the employee and ma be cause for denying <br />future employment by the City. The resignation of an employee who <br />fails to give notice shall be reported to the Personnel Director <br />immediately. <br />Section 23. Health and Welfare <br />23.1 The City will implement by March 1, 1996 an IRS 125 plan <br />which shall provide the following: <br />a. Pretax premium conversion of employee contribution <br />toward medical premiums. <br />b. Medical Flexible Spending Account with a maximum <br />employee pretax contribution of $1,000 per benefit <br />plan year. <br />C. Dependent Care Flexible Spending Account with a <br />maximum employee pretax contribution of $5,000 per <br />benefit plan year. <br />The City will implement a "core flex" medical benefit plan. <br />The core shall consist of the Kaiser Medical Plan with a <br />five dollar co -payment and the existing MetLife Dental Plan. <br />The City will contribute the following monthly <br />amount toward the election of benefits in the plan. <br />Employee $216.48 <br />Employee +1 $368.83 <br />Family $469.40 <br />Effective August 1, 1996, these amounts will be adjusted by <br />an amount equal to the change (increase or decrease) in the <br />Kaiser plan ($5 co -payment). <br />Employees electing to enroll in the Health Net or Blue Cross <br />Prudent Buyer Plan will be required to contribute the <br />difference between the premium for the Kaiser Plan and the <br />actual cost of the medical plan premium. Such contribution <br />shall be on a pre-tax basis. <br />Employees electing not to enroll in the core medical plan <br />(non enrollment in the medical and dental plan) and <br />demonstrate to the satisfaction of the City of enrollment in <br />another medical plan, shall receive $150 per month for their <br />20 <br />
