( Form Of ) <br />ADDENDUM TO <br />COORDINATING AGENCIES AGREEMENT <br />I. This is an Addendum (Addendum) to the Coordinating Agencies <br />Agreement (Agreement) entered into by and between Alameda County Health Care <br />Services (County Agency) and the City of Berkeley, Toxics Management Division; <br />Citv of Fremont Fire Department; City of Hayward; and City of Oakland (City <br />Agency(s)) (collectively, the Agencies) on or about January 28, 1997. <br />2. This Addendum adds the City of San Leandro as a "City Agency" to the <br />Agreement, and such addition has been required by the California Environmental <br />Protection Agency in order for the City of San Leandro to become a Certified Unified <br />Program Agency (CUPA). <br />3. The City of San Leandro shall be subject to all rights, privileges and <br />duties set forth in the Agreement, in the same manner as every other City Agency. <br />4. This Addendum may be signed in counterparts. <br />5. This Addendum shall be effective on the date it is fully executed by all <br />of the parties listed below. <br />The following individuals declare under penalty of perjury that they have <br />authority to sign this Addendum on behalf of the various Agencies and the CUPAs <br />those Agencies represent as designated in the Agreement. <br />
