EXHIBIT C <br /> <br />Following each forensic interview, a two-question survey is administered to children by <br />another staff member not involved in that child’s interview process. The questions are as <br />follows (for younger children, emoticon faces replace the word scale): <br /> <br />1. How did you feel about answering the interviewer’s questions today? CALICO staff made sure I understood the reason for our visit to CALICO. <br />3. When I came to CALICO, my child and I were greeted and received attention in a timely <br />manner <br />4. I was given information about the services and programs provided by CALICO. <br />5. My questions were answered to my satisfaction. <br />6. The process for the interview of my child at CALICO was clearly explained to me. <br />7. I was given information about possible behaviors I might expect from my child in the days <br />and weeks ahead. <br />8. The staff members at CALICO were friendly and pleasant. <br />9.
