LOCAL AGENCY FORMATION COMMISSION OF ALAMEDA COUNTY <br />APPLICATION QUESTIONNAIRE <br />I. GENERAL DATA AND BACKGROUND <br />1. If this proposal is made by Resolution of Application by the legislative <br />body of an affected city, county or district, attach two certified <br />copies of the resolution. <br />2. If this proposal is made by petition, attach a copy of the petition <br />to this application. Said petition should indicate if it is made <br />pursuant to and requests action under Government Code Section 35110 <br />or 56130. In addition, the petition should state the nature of the <br />proposal; include a boundary description and map; state reasons for <br />the proposal; whether signed by registered voters or landowners. If <br />desired, designate up to three petitioners with mailing addresses, <br />and the date each person signs the petition. (All signatures must <br />occur within six months of the first signature and be filed within <br />60 days after the last signature.) <br />3. If this is an "island annexation", attach a list and the mailing <br />addresses of all property owners, including public utilities, <br />within the boundaries of the proposal. <br />4. If this proposal is for incorporation of a city, formation of a <br />district or consolidation of cities or districts, give the proposed <br />name of the new agency. <br />S. <br />If the territory within the proposal is uninhabited, do each of the <br />appiicable property owners consent to the proposal? No. <br />If so, attach a certificate from the clerk of the legislative body <br />filing the application that the written consents are in his/her <br />possession. If by petition, a certificate from the applicable city <br />clerk that he/she has checked the number of owners and signatures <br />and that all have filed written consent is adequate evidence. <br />-2- <br />
