Consulting Services Agreement between City of San Leandro and Last revised 08/17/2020 Kimley-Horn and Associates, Inc. for Wicks Boulevard and Manor Boulevard Signal Upgrade Project Exhibit A – Page 5 of 6 Consultant will also support the City with preparing the draft RFA package at this stage to obtain E-76 authorization for construction from Caltrans local assistance. It is anticipated the City will furnish any applications and requirements to be used by the contractor to obtain permits for construction within city limits. Permit requirements will be included in an appendix to the technical specifications. Final plans, specifications, and estimates will be prepared following completion of the City’s plan check review. The final plans will be sealed by the professional engineer in responsible charge and will include the registration number and expiration date. The Final PS&E will be submitted to the City for use in advertising and constructing the improvements. Final plans will be provided electronically in PDF format and AutoCAD format on CD. Final specifications will be provided electronically in Microsoft Word format for the City’s use in finalizing the bid documents for advertisement. Final approval from PG&E is anticipated to be received during this task. Task 4.0 Deliverables: • Comment Resolution Matrix • Half-size pre-final plans in PDF format and six (6) 11"x17" hard copies • One set of 24"x36" Final Plans on Mylar • Final PS&E documents delivered to the City on CD • Plans in AutoCAD format • Specifications in Microsoft Word format • Estimate in Microsoft Excel format Task 5.0 Services Prior to Construction Prior to construction, during the bidding phase, Consultant will prepare any necessary addenda to the project plans. A total of 14 hours has been included for this task. Any additional effort will be considered as additional services to be completed for an additional fee upon receipt of written authorization from the City. Task 5.0 Deliverables: • Revised plan addendums Task 6.0 Services During Construction During construction, Consultant will review contractor submittals to check that they comply with the project plans and specifications. Consultant will conduct up to two (2) site visits during construction. In addition, Consultant will work with the City to prepare the elements of the award package to Caltrans local assistance that are pertinent to the technical design.
