<br /> <br />Consulting Services Agreement between City of San Leandro and Last revised 08/17/2020 <br />Kimley-Horn and Associates, Inc. for Wicks Boulevard and Manor Boulevard Signal Upgrade Project <br /> Exhibit A – Page 5 of 6 <br />Consultant will also support the City with preparing the draft RFA package at this stage to obtain E-76 <br />authorization for construction from Caltrans local assistance. <br /> <br />It is anticipated the City will furnish any applications and requirements to be used by the contractor to <br />obtain permits for construction within city limits. Permit requirements will be included in an appendix to the <br />technical specifications. <br /> <br />Final plans, specifications, and estimates will be prepared following completion of the City’s plan check <br />review. The final plans will be sealed by the professional engineer in responsible charge and will include <br />the registration number and expiration date. The Final PS&E will be submitted to the City for use in <br />advertising and constructing the improvements. Final plans will be provided electronically in PDF format <br />and AutoCAD format on CD. Final specifications will be provided electronically in Microsoft Word format for <br />the City’s use in finalizing the bid documents for advertisement. <br /> <br />Final approval from PG&E is anticipated to be received during this task. <br /> <br />Task 4.0 Deliverables: <br />• Comment Resolution Matrix <br />• Half-size pre-final plans in PDF format and six (6) 11"x17" hard copies <br />• One set of 24"x36" Final Plans on Mylar <br />• Final PS&E documents delivered to the City on CD <br />• Plans in AutoCAD format <br />• Specifications in Microsoft Word format <br />• Estimate in Microsoft Excel format <br /> <br />Task 5.0 Services Prior to Construction <br />Prior to construction, during the bidding phase, Consultant will prepare any necessary addenda to the <br />project plans. A total of 14 hours has been included for this task. Any additional effort will be considered as <br />additional services to be completed for an additional fee upon receipt of written authorization from the City. <br /> <br />Task 5.0 Deliverables: <br />• Revised plan addendums <br /> <br />Task 6.0 Services During Construction <br />During construction, Consultant will review contractor submittals to check that they comply with the project <br />plans and specifications. Consultant will conduct up to two (2) site visits during construction. <br /> <br />In addition, Consultant will work with the City to prepare the elements of the award package to Caltrans <br />local assistance that are pertinent to the technical design.
