<br />Consulting Services Agreement between <br />City of San Leandro and Jakes Associates Inc. for Project No. 2023.3400 Exhibit A – Page 1 of 1 <br />EXHIBIT A <br /> <br />SCOPE OF SERVICES <br /> <br /> <br /> <br />Project Description: <br /> <br />City of San Leandro owns and operates the Washington Manor Park Aquatic Center located at 14900 <br />Zelma St., which has been scheduled for rehabilitation. The Tower and Slide will be rehabilitated as part of <br />the project. <br /> <br />Description of Work: <br /> <br />Consultant shall provide an onsite visual inspection of Tower and slide for structural integrity to provide the <br />city with a recommendations report in regards to the viability of the structure to last for a <br />significant period without a complete remove and replace. <br /> <br />Tasks: <br />The following are the anticipated tasks for this contract: <br />1. Site visit <br />2. Perform inspection of slide fiberglass, connections, and yokes and components that relate to <br />the structural integrity of waterslide structure. <br />3. Document the inspection with images of the structure. <br />4. Review existing documentation <br />5. Conduct correspondence with O&M as needed <br /> <br />Deliverables: <br />A stamped report of inspection findings and engineering conclusion, including discussions related to in- <br />place repair vs removal and replacement of slide and tower. <br /> <br />Schedule: <br /> <br />Report to be completed and submitted within 5 business days of inspection. <br /> <br /> <br />DocuSign Envelope ID: F1AB1E0D-FD04-4CA8-8E43-F2B8EA28A468
