13 <br />Exhibit A <br />SCOPE OF SERVICES <br />BLOOD DRAW & EVIDENCE COLLECTION SERVICES <br /> <br />Contractor shall perform services related to the contracted Police Department Blood Drawing Program as <br />specified by the Director of Health Services. This includes the following specifications: <br /> <br />1. Qualified Certified Phlebotomy Technician or other qualified persons (pursuant to Vehicle Code 23158 <br />(A-L), Business and Professional Code sections 1242, 1242.5, 1246, available on a standby basis <br />seven (7) days per week, twenty-four (24) hours per day. <br /> <br />2. When requested by contracted Police Department Law Enforcement, the Director of Health Services, <br />Sheriff, District Attorney or their designated representatives, Vendor shall perform standard blood <br />drawing services in the Santa Rita Jail and/or local Hospital Emergency Room <br /> <br />3. When requested by contracted Police Department Law Enforcement, The Director of Health Services, <br />Sheriff, District Attorney or their designated representatives, Vendor will perform blood draw services <br />for HIV testing at the Laboratory at the County Crime Lab. The vendor is to provide all necessary <br />supplies for preparation and drawing of blood. <br /> <br />4. When requested by contracted Police Department Law Enforcement, the Director of Health Services, <br />Sheriff, District Attorney or their designated representatives, Vendor will perform blood draws and <br />evidence collection of patient samples as may be necessary for evidenti ary purposes in criminal <br />investigations. The Vendor is to provide all necessary supplies for preparation and drawing of blood <br />and obtaining patient specimen samples. <br /> <br />5. Vendor will at all times be in compliance with all laws regulating blood alcohol tests including Vehicle <br />Code Section 23158. If at any time Vendor is not in full compliance with all such laws, this contract can <br />be terminated immediately. <br /> <br />6. Vendor will comply with the “General Supervision” section of Vehicle Code 23158 (g), by maintaining <br />records and documentation of the following: “the supervisor of the technician is licensed under the <br />Business and Professions Code as a physician and surgeon, physician assistant, clinical laboratory <br />bioanalyst, registered nurse, or clinical laboratory scientist, and reviews the competency of the <br />technician before the technician may perform blood withdrawals without direct supervision, and on an <br />annual basis thereafter. The supervisor is also required to review the work of the technician at least <br />once a month to ensure compliance with venipuncture policies, procedures, and regulations. The <br />supervisor, or another person licensed as a physician and surgeon, physician assistant, clinical <br />laboratory bioanalyst, registered nurse, or clinical laboratory scientist, shall be accessible to the <br />location where the technician is working to provide onsite, telephone, or electronic consultation, within <br />DocuSign Envelope ID: C1BEC7AE-5DE9-4462-B82D-2CCCFD5A9C7F
