Exhibit A <br />Scope of Work <br /> <br />PROVIDER shall implement the FLEX RIDES On Demand Program, which provides low- <br />cost, fast, and convenient travel option through subsidized Transportation Network <br />Companies (TNCs) rides for members who need to travel to specific locations or <br />locations outside of San Leandro. <br /> <br />The RIDES On Demand Program includes: <br /> Eligibility: Registered FLEX RIDES members age 70+ OR Disabled Adults <br />age 18+ <br /> Service days/hours: 24 hours/day, 7 days/week <br /> Drivers will arrive within 5-15 minutes of call. <br /> Reservations: May be made up to 6 months in advance; however, ride <br />availability will be dependent upon driver availability at time of ride request <br />generated by Consultant. <br /> Fare: Rider pays first $4 of fare; City will pay up to $16 additional fare. Any <br />fare amount over $20 will be charged directly to the rider. <br /> Accessibility: Wheelchair Accessible Vehicles (WAVs) available but wait times <br />may be significantly longer dependent upon WAV driver availability at time of <br />ride request generated by Consultant. <br /> Service area: Alameda, Ashland, Castro Valley, Cherryland, Dublin, <br />Emeryville, Fremont, Hayward, Livermore, Newark, Oakland, Pleasanton, <br />San Leandro, San Lorenzo, San Ramon, and Union City. <br /> Maximum number of rides per rider per month: City will determine this <br />number based upon total ridership and will inform Consultant of any changes. <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br />
