Master Contract No. 902012 <br />Procurement Contract No. 24823 <br /> <br /> <br /> <br />EXHIBIT A <br />PROGRAM DESCRIPTION AND PERFORMANCE REQUIREMENTS <br /> <br />Contractor Name: City of San Leandro <br />Contracting Department: Health Care Services Agency (HCSA) <br />Office of Homeless Care and Coordination (OHCC) <br /> <br />Contract Period: 10/01/2022 – 06/30/2023 <br />Contract Amount: $500,091 <br /> <br />The City of San Leandro shall utilize HHAP funds towards the provision of the following services: <br /> <br />Operating subsidies in new and existing affordable or supportive housing units, emergency shelters, <br />non-congregate shelters, interim or bridge housing, and navigation centers. Operating subsidies may <br />include operating reserves. <br /> <br />Funding Overview: <br />HHAP guidelines specify spending must be informed by a best practices’ framework focused on moving <br />homeless individuals and families into permanent housing and ensuring those individuals and families <br />to maintain their permanent housing: <br />I. “Homeless,” as defined in Section 578.3 of Title 24 of the Code of Federal Regulations. <br />II. Housing-related activities funded with HHAP funds must be in compliance or otherwise aligned with <br />the Core Components of Housing First, pursuant to Welfare and Institution Code Section 8255(b). <br /> <br />The HHAP Grant Agreement is incorporated into this Agreement by this reference and may be relied <br />upon to interpret this Agreement. <br /> <br />The Target Population: <br />Provide low-barrier, compassionate, and non-judgmental shelter and case management to individuals <br />experiencing homelessness; assistance to rapidly secure and obtain permanent housing. <br /> <br />Program Names and Locations: <br />Sister Me Home - (Interim Housing) <br />site location not disclosed to ensure confidentiality <br />Central office location: 1840 Fairway Drive, San Leandro, CA 94577 <br /> <br />San Leandro Shelter - (Interim Housing) <br />Saint Leander’s Parish <br />501 Davis St. San Leandro, CA 94577 <br /> <br /> <br /> <br /> <br />Exhibit A <br />Page 1 of 8
