<br />Non-Professional Services Agreement between July 1, 2023 <br />City of San Leandro and Advanced IPM —Exhibit A Page 1 of 2 <br />EXHIBIT A <br /> <br />SCOPE OF SERVICES <br /> <br />The scope of work of this contract will include all materials, transportation, labor, equipment, any and all <br />services and materials necessary to perform pest control services and provide a pest-free environment in <br />specified offices, buildings and/or grounds. <br /> <br />Contractor must be a qualified pesticide applicator (operator) business duly registered and licensed in the <br />State of California to render such services will be considered. <br /> <br />Contractor is to provide for an environment that is free from, but not limited to, the following pests: <br /> <br />1. Ants and cockroaches <br />2. Spiders <br />3. Fleas and other biting insects <br />4. Bed Bugs <br />5. Termites <br />6. Wasps, hornets, and other stinging insects nesting in City facilities and/or parks <br />7. Mice, rats, and other rodents <br /> <br />All buildings and properties listed on the Compensation Schedule (Exhibit B) shall receive monthly services <br />for ants and roaches. In addition, all buildings will receive service for spiders at least one time per year. <br />Specified facilities will receive monthly, bi-monthly or quarterly treatment for bed bugs, and rodents. <br /> <br />The City reserves the right to add or remove facilities listed on the Compensation Schedule (Exhibit B) at any <br />time, for any duration, and for any reason, during the agreement term. The Contractor will be required to <br />quote for service for any additional facilities as requested by the City. Furthermore, the Contractor will be <br />required to deduct costs for any facilities where services is not provided as specified in the Compensation <br />Schedule. <br /> <br />All Contractor personnel shall wear a uniform with proper identification whenever working in or around any <br />City facility and service vehicles shall be clearly marked. Contractor personnel shall schedule visits with the <br />Building Supervisor prior to providing services and check in with facility staff prior to performing any inspection <br />or service. The contractor must perform a thorough inspection during routine service. <br /> <br />All pest control services shall be performed in accordance with Federal, State, and local laws and regulations. <br />Any chemicals used during the service must be approved for its intended use and applied in a manner <br />consistent with the regulations established by the State of California, Department of Pesticide Regulation and <br />consistent with Federal and Cal OSHA standards. <br /> <br />The Contractor shall adhere to the City’s Integrated Pest Management (IPM) Policy and take special care to <br />protect the public from any exposure to pest control traps, bait stations, and chemicals. <br /> <br /> <br /> <br />DocuSign Envelope ID: 9482C4C4-6521-4E22-83CE-E36814FF64EB
