Page 2 of 4 <br />• Eligibility: Registered FLEX RIDES members age 70+ OR Disabled Adults age 18+ <br />• Service days/hours: 24 hours/day, 7 days/week <br />• Drivers will arrive within 5-15 minutes of call. <br />• Reservations: May be made up to 6 months in advance; however, ride availability will be <br />dependent upon driver availability at time of ride request generated by Consultant. <br />• Fare: Rider pays first $4 of fare; City will pay up to $16 additional fare. Any fare amount over <br />$20 will be charged directly to the rider. <br />• Accessibility: Wheelchair Accessible Vehicles (WAVs) available but wait times may be <br />significantly longer dependent upon WAV driver availability at time of ride request generated by <br />Consultant. <br />• Service area: Alameda, Ashland, Castro Valley, Cherryland, Dublin, Emeryville, Fremont, <br />Hayward, Livermore, Newark, Oakland, Pleasanton, San Leandro, San Lorenzo, San Ramon, <br />and Union City. <br />• Maximum number of rides per rider per month: City will determine this nu mber based upon <br />total ridership and will inform Consultant of any changes. <br /> <br /> <br /> <br />Exhibit B <br />Compensation Schedule & Reimbursable Expenses <br /> <br />Funding for the City of San Leandro Paratransit Program is entirely contingent upon successfully receiving <br />Measure BB funding from the Alameda County Transportation Commission. <br />Compensation for services rendered is based on active rider fees, per ride subsidy and Consultant’s fee <br />per minute in vehicle. <br />Per Rider Per Month (PMPM) Fee 0 - 250 Riders $9.00/month each - <br />Minimum Flat Fee $900.00/month if there are less <br />than 100 Riders in any month <br />*Per Ride Subsidy $16.00 per ride <br />Consultant’s Fee per Minute in Vehicle $0.27 per minute <br /> <br />*Rider is solely responsible for first $4 of the fare plus any amount in excess of $20.00. <br />Compensation for all costs rendered is based on fees noted above, along with annual cost projections based <br />on ridership data. <br />Projected Annual Cost for Consultant’s Fee per <br />Minute in Vehicle <br />$15,425 <br />Projected Annual Cost for Active Rider Fee $12,500 <br />Projected Annual Cost for Per Ride Subsidy $65,280 <br />Total for Projected Costs for Two-Year <br />Extension <br /> <br />$186,410 ($93,205/year) <br />DocuSign Envelope ID: 524467E2-1FFD-4F62-AD21-E9B5963F13DD
