<br />Consulting Services Agreement between City of San Leandro and Last revised 6/16/2023 <br /> SAVE for CAP Grant: Crisis Response Team Exhibit A – Page 1 of 2 <br />EXHIBIT A <br /> <br />SCOPE OF SERVICES <br /> <br /> <br />SAVE's Crisis Response services aim to assist individuals who have experienced intimate partner violence <br />and their families in breaking the cycle of violence. The services provided include crisis counseling, <br />domestic violence education, case man agement, and restraining order assistance. The Crisis Response <br />advocates ensure that survivors referred by the San Leandro Police Department or other local service <br />providers receive crisis intervention support. Additionally, they play a crucial role in pre venting survivors <br />from becoming homeless by connecting them with SAVE's housing program for obtaining or maintaining <br />permanent housing. Survivors are also referred to SAVE's counseling services for individual or group <br />support. SAVE works towards removing b arriers and assisting immigrants, persons with disabilities, and <br />non-English speakers in accessing eligible services. <br />Survivors of domestic violence receive immediate and comprehensive support to address and recover from <br />abuse. Community members are also p rovided with information about additional support services available. <br />The Crisis Response services target approximately 100 low -income survivors of violence each year, with at <br />least 50% of them engaging in the provided services. <br /> <br />Services: <br />1. Crisis counseling <br />2. Domestic violence education <br />3. Case management <br />4. Restraining order assistance <br />5. Emergency Response (support with basic needs) <br /> <br />Outcome: <br />1. A combination of outreach to and receiving 100 Alameda County residents referred to SAVE as <br />domestic violence survivors. Offer crisis intervention, peer counseling, domestic violence <br />education, legal system navigation, and resource assistance. <br /> <br />Data from program participants is collected during service provision and stored securely in a customized <br />database called ETO from Social Solutions. This database enables the extraction of aggregate data for <br />reporting purposes. <br />The Crisis Response Team consists of staff members who answer the crisis line, respond to law <br />enforcement calls, and provide comprehensive services to survivors seeking shelter, safety planning, <br />restraining order assistance, and immediate support. <br />DocuSign Envelope ID: 66BDF303-B87E-4A5A-AF56-E4E3883421CC
