<br /> <br />Consulting Services Agreement between City of San Leandro and Seneca Family of Agencies Last revised <br />06/16/2023 <br />for Cap Grant: Pathways Counseling Center Exhibit A – Page 1 of 1 <br />EXHIBIT A <br /> <br />SCOPE OF SERVICES <br /> <br /> <br />This project aims to improve the wellbeing and mental health of families in San Leandro's least resourced <br />neighborhoods. It provides counseling and case management services to children affected by abuse, <br />neglect, loss, disrupted attachment, and adverse childhood experiences, as well as other challenges like <br />poverty, housing instability, food insecurity, violence exposure, social isolation, and family mental illness or <br />substance use. The goal is to serve all families in need within the target population. <br />Services Provided: <br />1. Individual, Family, and/or Group Therapy: Using Trauma -Focused Cognitive Behavioral Therapy (TF- <br />CBT), children receive therapy to reduce trauma symptoms, strengthen family bonds, improve emotional <br />regulation, and promote parental support. Therapy is based on clinical assessments and treatment plans. <br />2. Crisis Intervention & Safety Planning: Clinicians provide crisis support during therapy sessions and help <br />families create Safety Plans for crisis management. Malabar House offers voluntary respite stays with <br />mental health sessions at the beginning and end. <br />3. Case Management: Individualized services coordinate resources such as housing, after -school <br />programs, legal support, and mentoring to meet specific client needs. <br />Goals and Objective: <br />1. Number of Youth Served: On average, 3 youth and their families are served at a time, with 6-8 youth <br />served annually. <br />2. Progress Toward Treatment Goals: 80% of youth discharged should demonstrate progress through <br />changes in actionable items on their Alameda County Child Adolescent Needs and Strengths (CANS) <br />assessments over time. <br /> Assessment Tools: <br />1. Alameda County Child Adolescent Needs and Strengths (CANS) Ages Birth to 24: Collaborative <br />assessments evaluate youth and family functioning in multiple core domains. CANS is conducted at intake, <br />every six months, and at discharge to measure the impact of program services. <br />2. Child Posttraumatic Symptom Scale (CPSS) and CPSS -Caregiver Version: Self-administered scales <br />assess trauma-related symptoms based on the TF-CBT protocol. <br />3. Pediatric Symptom Checklist (PCS-35): Clinician-collaborated checklist highlights developmental <br />strengths and concerns, administered at intake, every six months, and at discharge. <br />4.
