This is a two-page document. Please carefully review the attached Special Waste Service <br />Agreement as well as the terms and conditions on page 2. <br /> <br />Please follow the instructions below prior to scheduling and shipping material to <br />the landfill: <br /> <br /> Sign and date the Service Agreement in the space provided in the lower bottom left corner of <br />the Service Agreement. <br /> Initial and date the terms and conditions page in the lower left corner of the page. <br /> Return the service agreement via email to the sender or Fax to: 209-466-1067. <br /> Failure to return properly executed documents in a timely manner could delay our ability to <br />provide you with excellent service. <br />Scheduling Shipments & Non-Hazardous Waste Manifests: <br /> Please schedule large quantity shipments 24 to 48 hours in advance. <br /> Each load must be accompanied by a signed and dated Republic Services Non-Hazardous <br />Waste Manifest. <br /> Republic Services will pre-print and supply manifests to you. <br /> If you have not received a manifest, please request manifests for your project by contacting <br />Patricia Moreno at 800-204-4242 or via email: pmoreno@republicservices,com <br /> A “wet” signature and shipment date is required by the authorized signatory on each manifest. <br /> Any loads that arrive without a wet signature or the proper documentation will be rejected. <br />Please call your representative with any questions concerns.
