EXHIBIT A - Page 1 of 2 <br />EXHIBIT A <br />OVERVIEW OF SERVICES <br />Mercy Retirement and Care Center (“MRCC”) and the City are partnering to provide food to low-income <br />senior citizens living at San Leandro. <br />MRCC agrees to the following: <br />1.MRCC shall provide the MBBP at no cost to City patrons. <br />2.MRCC shall do its best to ensure freshness and quality of food distributed to local seniors <br />through MBBP. <br />3.MRCC shall deliver ongoing food items in bulk on the second and fourth Monday of each <br />month at the following participating location: <br />San Leandro Senior Community Center (SCC) <br />13909 E 14th Street <br />San Leandro, CA 94578 <br />4.MRCC shall deliver ongoing food items in bulk via it’s Mobile Pantry on the second and fourth <br />Tuesday of each month at the following participating location: <br />San Leandro Marina Community Center (MCC) <br />15301 Wicks Boulevard <br />San Leandro, CA 94579 <br />5.MRCC shall provide the City the necessary paperwork, flyers, and/or posters as needed, either <br />hard copy or electronic to facilitate operation and promotion of the MBBP, and promotion of <br />healthy food choices as prescribed by the USDA. <br />6.All MRCC staff and volunteers responsible for providing and delivering the food shall follow all <br />applicable state and county public orders and comply with public policy and procedures. <br />7.MRCC shall maintain and follow a food safety plan and train its staff in proper food safety. <br />8.MRCC staff will provide training in the correct processes to record and document receipt and <br />distribution of food. <br />9.MRCC is committed to protecting the health of our community and preserving the safety of your <br />residents and staff. For that reason, MRCC will follow any rules or guidelines issued by the <br />CDC or the Alameda County Health Department in regard to social distancing, masking, and <br />employees who test positive or are symptomatic of COVID-19 or RSV. <br />The City agrees to the following: <br />1.Provide a handicapped-accessible, well-maintained space large enough for the Mobile Pantry <br />itself with additional space for staff and recipient seniors to comfortably maneuver around the <br />Mobile Pantry. <br />2.Provide access to a handicapped-accessible, well-maintained bathroom that is easily accessed <br />from the space provided for the Mobile Pantry. <br />3. Support resident participation through outreach and publicizing the program on flyers and <br />monthly calendars. <br />4.Ensure USDA information poster (i.e. “And Justice For All”, “EFA14 TEFAP Income Guideline <br />2023” and “Mercy Brown Bag Program Grievance Policy”) is displayed at every distribution <br />(MBBP shall provide). <br />5.The City shall be responsible to coordinate the logistics of the food distribution, answer <br />questions and oversee the MBBP operations as necessary. <br />6.The City shall be responsible for confirming receipt of delivery by signing MRCC Proof of <br />DocuSign Envelope ID: B7D431A8-D5C5-4F91-8EE2-BFC745D33909
