<br />Consulting Services Agreement between City of San Leandro and November 3, 2023 <br />Construction Testing Services, Inc. Exhibit A – Page 1 of 2 <br />EXHIBIT A <br /> <br />SCOPE OF SERVICES <br /> <br />The Consultant shall perform Special Inspection services and submittal reviews related to the third and final <br />phase of the Police Building and South Offices Modifications Project. <br /> <br />The duties and responsibilities of the Consultant shall include: <br />- Observation of work for conformance with the project plans and specifications and applicable <br />workmanship provisions of the California Building Code <br />- Observation/testing of items marked on the Special Inspection and Testing Schedule on the <br />following page of this Agreement <br />- Reporting of nonconforming items to the immediate attention of the contractor and noting of such <br />items in the daily report. If any item is not resolved in a timely manner or is about to be <br />incorporated into the work, the Consultant shall immediately notify the project engineer or architect <br />and the City of San Leandro Building and Safety Division at (510) 577-3405 or in person, and post <br />a discrepancy notice. <br />- Review of contractor submittals related to work that requires Special Inspections <br /> <br />Timeline: The City will coordinate with the Consultant regarding the dates and times when Special <br />Inspection services are to be provided. <br /> <br />DocuSign Envelope ID: 04A45B4F-984A-4CC6-BAE3-B1665DA06944
