<br /> <br /> <br />Consulting Services Agreement between City of San Leandro and Last revised 2/14/2024 <br />V&A CONSULTING ENGINEERS for CONCRETE STRUCTURES CONDITION ASSESSMENT Exhibit A – Page 1 of 3 <br />EXHIBIT A <br /> <br />SCOPE OF SERVICES <br /> <br />Task | Description <br />1. Project Management: Track and execute the project in accordance with the schedule, budget, <br />and quality expectations that are established. The project duration is assumed to be 3 <br />months. This task includes the following project management work activities: <br />a. Monitor project progress, including work completed, work remaining, budget <br />expended, schedule, estimated cost of work remaining, and estimated cost at <br />completion; manage activities within the total project budget. <br />b. Monitor project activities for potential changes and anticipate changes whenever possible; <br />with approval, modify project tasks, task budgets, and approach to keep the overall project <br />within budget and on schedule. <br />c. Manage the quality of all work activities and project deliverables. <br />2. Document Review and Preparations: Review the drawings and existing documentation for the <br />structures to prepare for the assessment. Prepare the testing equipment necessary to perform <br />the assessment. For the confined space entry assessment of Digester 4, V&A will follow Cal <br />OSHA protocols in the field and will prepare a confined space entry permit; however, a safety <br />plan will not be submitted. <br />3. Visual Assessment: Perform a high-level visual assessment of the structures. A total of 12 <br />structures will be evaluated during a one (1) day site visit, assuming that the structures will <br />be available during the site visit. Confined space entry will not be performed. The assessment <br />will be performed by two V&A personnel. The assessment will include the following: <br />a. Visual Assessment: Qualitative condition assessment observations are subjective and <br />based upon the evaluator’s expertise. <br />i. Visually assess safely accessible surfaces of the structures. <br />ii. Rate conditions using the VANDA® Concrete Index. See attached. <br />iii. Document observations using digital photographs. A minimum of 10 photos will be <br />obtained from each structure. <br />4. Detailed Assessment: Perform follow-up detailed assessments of concrete conditions based on <br />the results of the visual assessment performed in Task 3. V&A will provide a list of structures <br />to be assessed in detail. Provide two (2) V&A engineers and testing equipment for one (1) day <br />to collect detailed information that will help characterize the extent of concrete degradation, as <br />accessible , without performing confined space entry. Provide four (4) V&A engineers and <br />technicians to perform a detailed confined space entry assessment of Digester 4. The following <br />methods will be performed, as applicable, on a structure-by-structure basis. <br />i. Measure the thickness of concrete protecting the reinforcing steel using surface <br />penetrating radar (SPR) at up to 4 easily accessible locations in each structure to be <br />assessed. <br />ii.
