<br /> <br /> <br />Consulting Services Agreement between City of San Leandro and Last revised 2/14/2024 <br />V&A CONSULTING ENGINEERS for CONCRETE STRUCTURES CONDITION ASSESSMENT Exhibit A – Page 3 of 3 <br />Assumptions <br />The following is a list of additional assumptions used to develop V&A’s scope of work. <br />▪ V&A will submit monthly invoices in electronic format via email. Time spent submitting the <br />invoice via a different method may incur additional charges. <br />▪ Confined space entry will be performed in Digester 4, which is currently out of service and <br />drained. V&A assumes that there is no residual water or sludge at the bottom of the digester <br />that would prevent safe access to the digester interior. V&A also assumes that the City will <br />open the side manways and roof manway(s) prior to V&A’s arrival to the site. V&A also <br />assumes that the City will perform lock-out, tag-out (LOTO) of all energy sources that could <br />affect the safety of entrants. V&A will verify LOTO with the City at the site and place locks on <br />the LOTO box prior to entering the digester. <br />Prevailing Wage Assumption <br />V&A understands some or all tasks associated with this project are subject to prevailing wage rate <br />requirements. All project labor subject to prevailing wage requirements will incur a 30% markup on <br />the associated labor fee. This markup covers the higher base hourly labor rates associated with <br />prevailing wage rates, additional premium pay requirements, and reporting requirements. <br />Exclusions and Limitations <br />The following items, unless otherwise indicated, are not included in the scope of work: <br />▪ Project Specific Health and Safety Plan <br />▪ Permitting <br />▪ Notification <br />▪ Bonds <br />▪ Shutdown, Dewatering, and Cleaning of Structures <br />▪ Excavations <br />▪ Confined Space Entry <br />▪ Supplied Air <br />▪ Vendor Portal Registration <br />▪ Payment Portal Invoice Submission <br />DocuSign Envelope ID: B1FEB12C-3187-4688-9A64-AA6A32699F6E
