<br />Amendment No. 1 to <br />Consulting Services Agreement between City of San Leandro and VACCEB 04/09/2024 <br />For Bi-lingual Information & Assistance Program for Seniors Page 1 of 4 <br />AMENDMENT NO. 1 TO CONSULTING SERVICES AGREEMENT BETWEEN THE CITY OF SAN LEANDRO AND <br />VIETNAMESE AMERICAN COMMUNITY CENTER OF THE EAST BAY (VACCEB) <br />FOR <br />SENIOR SERVICES <br /> <br />This Amendment No. 1 (“Amendment”) is made by and between the City of San Leandro (“City”) <br />and VACCEB (“Consultant”) (together sometimes referred to as the “Parties”) as of April 9, 2024, and <br />amends that certain Consulting Services Agreement (“Agreement”) dated November 1, 2023, between the <br />Parties. <br /> <br />WHEREAS, City and Consultant have executed the Agreement, pursuant to which Consultant has <br />provided certain consulting services to City with regard to Programs to Seniors, and <br /> <br />WHEREAS, the Parties desire to amend the Agreement to update the Scope of Services. <br /> <br />NOW THEREFORE, for good and valuable consideration, the sufficiency of which is hereby <br />acknowledged, the Parties hereby agree to amend the Agreement as follows: <br /> <br /> <br />1. Exhibit A of the Agreement entitled “Scope of Services” is hereby amended to read: <br /> <br />1. Bilingual Information & Assistance Program to Seniors (defined as 62 years old and over) in languages <br />including but limited to: Vietnamese, Chinese, Amharic, and Spanish. Up to 250 clients served during <br />the contract year. Following service areas included: <br /> <br />a. CalFresh/Food Insecurity <br />b. Housing/Rental Issues <br />c. Legal Referral <br />d. Healthcare (Medicaid, Medicare, Insurance) <br />e. Domestic Violence <br />f. Mental Health Referral <br />g. Social Security Administration <br />h. Household (Telephone/Utility Bills/Disputes) <br />I. Safety and Injury Prevention <br />j. Employment/Unemployment Benefit Inquiries <br />k. Advance Health Care Directives <br /> <br />2.Three (3) Citizenship classes taught on a quarterly basis (e.g. Jan-Mar; Apr-Jun; Jul-Sep.). Up to 75 <br />students served during the contract year. <br />3. Operating Hours at the Senior Community Center: Monday, Tuesday, Thursday, Friday: 9am-1pm <br />4. Outreach – With assistance from the City of San Leandro, VACCEB will perform bi-lingual outreach to <br />San Leandro residents offering above services. Host two “Meet and Greet” events during contract period. <br />5.
