during the same hours as ADA reservation service, vehicles used will be <br />the same as ADA service, the level of service to be provided (door-to- <br />door, assistance with packages, etc.) and the service area will be the <br />same. Ride requests for travel between the hours of 4:00 am and <br />midnight will be accepted. <br />6. Service will not be provided if its provision will have a negative irnpact <br />upon the service quality of the Consortium's ADA trips. <br />7. The broker will bill the City of San Leandro $18.44 per trip, which is 50% <br />of the average cost of providing ADA paratransit trips by the Consortium. <br />The Consortium has agreed to this reduced cost to allow the City of San <br />Leandro to maintain service while planning the use of the Measure B and <br />to reflect the fact that some of each Measure B trip provided will be within <br />the ADA service area. <br />8. No charge will be assessed for escorts and personal care attendants <br />accompanying the ADA rider in whose name the reservation was placed. <br />9. Invoices will be generated by the broker once a month, by the 20th of the <br />month following the month of service. Invoices will include the name, <br />address, and time of the non-ADA trip performed. Invoices will be sent <br />by the broker to the City of San Leandro Paratransit Program Manager for <br />payment. <br />10. The City of San Leandro will reimburse the broker within 30 days from <br />receipt of an acceptable invoice. <br />11. The City of San Leandro may elect to terminate this agreement at any <br />time, with two weeks notice. <br />12. The Broker may terminate this agreement at any time, with 90 days notice <br />to the City of San Leandro. <br />13. The Broker shall ensure that service providers procure and maintain for the <br />duration of the services insurance against claims for injuries to persons or <br />damages to property which may arise from or in connection with the provision <br />of paratransit services pursuant to this MOU. The City of San Leandro shall <br />be added as an "additional insured" on provider insurance policies. Providers <br />shall maintain limits no less than $1,000,000 General Liability: combined <br />single limit per occurrence for bodily injury, personal injury and property <br />damage. <br />2
