Facility Information (Forms) <br />Proposers shall complete all facility information required in the “Transfer” Tabs of Attachment A. <br />Please see Attachment A. <br /> <br />2. Facility Documents (Attachments) <br />Proposers shall provide the following attachments for each facility: <br />a. A copy of the Solid Waste Facility Permit <br />Please see the following pages for the Solid Waste Permit for DSTS.
