Consulting Services Agreement between City of San Leandro and Last revised 12/12/2024 Davis Street Family Resource Center for Community Food Services Exhibit A – Page 1 of 1 EXHIBIT A SCOPE OF SERVICES Davis Street Family Resource Center services address the increasing demands of our clientele, which includes low-income adults, seniors, children, youth, individuals with disabilities, and homeless families. More than 85% of those we assist live at or below 30% of the federal poverty level. Clients enrolled in our program receive a pre-packaged bag containing 3 to 5 days' worth of food, including fresh fruits and vegetables, canned goods, meat, dairy (when available), and other items donated by the community. While the food pantry remains the cornerstone of the Basic Needs program, we also offer housing, rental, and utility assistance, transportation support to access the pantry, mental health counseling, medical and dental care, supportive services for individuals with disabilities, and various information and referral services, all while maintaining the original programs that have been the foundation of Davis Street for over 50 years. Davis Street’s Basic Needs program features a food pantry operating year-round from Monday to Friday 10am – 5pm, serving over 12,000 individuals each year. Davis Street Family Resource Center (Consultant) shall use funding to cover a portion of the Intake Specialist salary and benefits, as well as food and food bag purchases. Consultant will distribute food to low-income families facing food insecurity. Reporting requirements: Reporting: Biannual reporting (Due July 8, 2025, and January 31, 2026) and invoicing.
