<br />Consulting Services Agreement between City of San Leandro and Last revised 12/12/2024 <br />Korean Community Center of the East Bay for Community Food Services Exhibit A – Page 1 of 1 <br />EXHIBIT A <br />SCOPE OF SERVICES <br /> <br /> <br />Give a Meal of Love (GML) Program <br /> <br />GML program to provides culturally resonant and nutritious meals to low income and highly isolated seniors <br />in a culturally welcoming and dignified way. During meal delivery, Consultant’s Korean and Chinese <br />speaking staff provide wellness check-ins, using this as an opportunity to connect seniors to social <br />services, counseling, and wellness and senior leadership programs as needed. In doing so, GML also <br />provides essential physical and mental health resources and social connection. <br /> <br />Consultant will use funding to partially fund GML program which will provide and deliver nutritious, culturally <br />relevant meals to seniors three times per week. Funding will also be used to partially fund the salaries of <br />the Program Manager and Community Engagement Manager. <br /> <br /> <br />Reporting requirements: <br />Reporting: Annual reporting (Due July 8, 2025) and invoicing. Total unduplicated individuals served, <br />disaggregated by: <br />• Race and ethnicity <br />• Income status (extremely, low, moderate) <br />• Gender <br />• Units of food distributed (e.g., number of meals, pounds of food, etc.) <br />Docusign Envelope ID: 6AD8065D-CF56-403A-8B06-493F0708E24B
