<br />Consulting Services Agreement between City of San Leandro and Last revised 12/12/2024 <br />SOS Meals on Wheels for Community Food Services Exhibit A – Page 1 of 1 <br />EXHIBIT A <br /> <br />SCOPE OF SERVICES <br /> <br />SOS Meals on Wheels (Consultant) delivers meals to seniors 248 days per year, Monday through Friday, <br />with weekend meals delivered with Thursday and Friday meals. <br /> <br />Funding is to help cover distribution of 55,000 meals to San Leandro homebound seniors from January 1, <br />2025, to December 31, 2025. Specifically, funding is to be used for raw food, packaging, mileage <br />reimbursement, and Volunteer Coordinator salary. <br /> <br />Reporting requirements: <br />Reporting: Biannual reporting (Due July 8, 2025, and January 31, 2026) and invoicing.
