<br />Consulting Services Agreement between City of San Leandro and Last revised 12/12/2024 <br />Vietnamese American Community Center of the East Bay for Community Food Services Exhibit A – Page 1 of 1 <br />EXHIBIT A <br />SCOPE OF SERVICES <br /> <br /> <br />Food Security Program: which includes both Consultant’s congregate (hot meals) and meal bag programs <br />(for fresh fruits and vegetables). <br /> <br />Consultant’s Program Manager and Program Coordinator collect, compile, and evaluate data from each <br />program to ensure goals and contract compliance are met. Clients who register for programs are vetted to <br />confirm they meet the age and low-income requirements. Additionally, clients are surveyed annually to <br />gather information and feedback, facilitating community engagement to ensure their needs are being met <br />and to gather suggestions for program improvements. <br /> <br />Consultant will use funding in part for staffing costs (Program Manager and Program Assistant) and <br />primarily for food costs to support Consultant’s Food Security Program, which encompasses both the hot <br />meal and meal bag (fresh fruits and vegetables) programs. Funding will support an estimated 1,140 meals. <br /> <br />Reporting requirements: <br />Reporting: Annual reporting (Due July 8, 2025) and invoicing.
