Consulting Services Agreement between City of San Leandro and Last revised 12/12/2024 Tibrucio Vasquez Health Center for Community Food Services Exhibit A – Page 1 of 1 EXHIBIT A SCOPE OF SERVICES Food Distribution: Partnering with the Alameda County Community Food Bank since 2020, TVHC’s (Consultant) Community Outreach and Advocacy (COA) team has organized and implemented food distributions events rotating weekly at Consultant’s health centers across their service area at Mission Health Center in Hayward, Ashland Health Center in San Leandro, Union City Health Center in Union City, and formerly Firehouse Clinic in Hayward. Food distribution events are staffed by Consultant’s COA team and promotoras (stipend volunteers) and serve 400-500 families each week. During food distribution, Consultant’s team also educates the community about TVHC and other community services, refers them to Consultant’s clinics, and helps them sign up for health insurance and other public benefits. Consultant shall use funds to sustain their existing food security program. Funds will primarily be used to fund staffing of the Community Impact Coordinator, as well as other indirect costs. Reporting requirements: Reporting: Biannual reporting (Due July 8, 2025, and January 31, 2026) and invoicing.
