<br />Consulting Services Agreement between City of San Leandro and Last revised 12/12/2024 <br />Tibrucio Vasquez Health Center for Community Food Services Exhibit A – Page 1 of 1 <br />EXHIBIT A <br />SCOPE OF SERVICES <br /> <br /> <br />Food Distribution: Partnering with the Alameda County Community Food Bank since 2020, TVHC’s <br />(Consultant) Community Outreach and Advocacy (COA) team has organized and implemented food <br />distributions events rotating weekly at Consultant’s health centers across their service area at Mission <br />Health Center in Hayward, Ashland Health Center in San Leandro, Union City Health Center in Union City, <br />and formerly Firehouse Clinic in Hayward. Food distribution events are staffed by Consultant’s COA team <br />and promotoras (stipend volunteers) and serve 400-500 families each week. During food distribution, <br />Consultant’s team also educates the community about TVHC and other community services, refers them to <br />Consultant’s clinics, and helps them sign up for health insurance and other public benefits. <br /> <br />Consultant shall use funds to sustain their existing food security program. Funds will primarily be used to <br />fund staffing of the Community Impact Coordinator, as well as other indirect costs. <br /> Reporting requirements: <br />Reporting: Biannual reporting (Due July 8, 2025, and January 31, 2026) and invoicing.
