<br />Consulting Services Agreement between <br />City of San Leandro and Jakes Associates Inc. for Project No. 2023.3400 Exhibit A – Page 1 of 2 <br />EXHIBIT A <br /> <br />SCOPE OF SERVICES <br /> <br /> <br />Project Description: <br /> <br />City of San Leandro owns and operates the Washington Manor Park Aquatic Center located at 14900 <br />Zelma St., San Leandro, CA which is undergoing various Improvements. <br /> <br />Description of Work: <br /> <br />Consultant shall provide on call services to attain necessary documentation and permits to reopen pool to <br />the public. The scope of work is expected to provide the following Deliverables and may be expanded on <br />an on call basis. <br /> <br />Deliverables: <br /> <br /> General: Documents in this section will need to be signed/stamped by a licensed engineer <br /> <br />1. Notify the Division, in writing, at least 30 days prior to opening the ride to the public. <br />a. The notification shall state the location of the ride, the date the owner or operator <br />intends to commence public operation, and the earliest date the ride will be ready for <br />inspection by the Division. <br />2. A written certification from a licensed engineer that the ride as modified meets all applicable <br />design requirements set forth in Subchapter 6.2 of Chapter 4 of Division 1 (starting at section <br />3195.1) of this Title. <br />3. Documentation to satisfy existing condition or as-builts for approval from calosha. <br />a. Existing as-builts and unfinished summary of recent construction shall be provided. <br /> Submittals: <br /> <br />1. Schedule (Initial Inspection and target dates) <br />2. Task List of corrections (provide details needed to complete correction) <br /> <br /> The following are the anticipated tasks for this contract: <br /> <br />1. Site visit/Inspection of structure(s) <br />2. Provide a task list of any corrective work <br />3. Coordination and correspondence to make structure compliant to current regulatory <br />agency such as Calosha. <br />4. Confirmation of completion of corrective work for as-builts <br />5.
