<br />Consulting Services Agreement between City of San Leandro and Last revised March 2025 <br />Freyer & Laureta, Inc. for San Rafael St. Force Main Replacement Exhibit A – Page 1 of 1 <br />EXHIBIT A <br /> <br />SCOPE OF SERVICES <br /> <br />Project Description <br />The existing 6-inch diameter cast iron (CI) pipe from the San Rafael Lift Station services <br />approximately 100 properties between San Rafael Street and Grand Avenue. Extending <br />approximately 700 feet to its discharge point on Sybil Avenue, the pipe has deteriorated <br />significantly due to corrosion and has reached the end of its service life. A full replacement is <br />now required. <br /> <br />Scope of Services <br />The consultant shall provide engineering and surveying services for the <br />replacement/rehabilitation of the force-main and the construction of a new bypass valve on San <br />Rafeal Street. The tasks involved for this service shall include: <br /> <br />Task 1 – Project Management and Coordination <br />• Freyer & Laureta (F&L) will attend meetings for project coordination. Three (3) <br />meetings are budgeted. <br />• F&L will provide QA/QC and monitor schedule and budget. <br /> <br />Task 2 – Site Investigation, Data Collection, and Research Study <br />• Update and resubmit the bid package. <br />• Provide written responses to submitted Bidder’s questions. <br />• Prepare & provide Addenda documents as needed for the project. <br />• Assist City with Bid evaluations. <br /> <br />Task 5 – Engineering Services During Construction <br />• The consultant will provide construction support services to ensure work is completed <br />insubstantial conformance with the contract documents. These services will consist of the <br />following: <br />a. Interpretating contract documents, reviewing extra work requests, reviewing <br />shop drawings and other contractor submittals, and reviewing and responding <br />to requests for information/clarifications. <br />b. Conducting up to four (4) site visits to resolve field issues, as needed.. <br />c. Attending up to four (4) weekly construction meetings. <br />d. Providing project close-out support, including a site visit for punch list <br />documentation. <br />e.
