That means having access to free or low-cost health and <br />wellness services, in-home assistance, and health education. <br />2 Provide respite and resources for family and <br />volunteer caregivers <br />Project Lead: Human Services Department <br />Action Step #1. Convene local and regional public and <br />nonprofit caregiving, in-home supportive services, and <br />adult daycare providers to assess the level of unmet <br />demand for these services. Calculate the cost of expanding <br />each program to meet the demand. <br />Action Step #2. Explore implementing a local project of <br />the Village to Village program, which relies on volunteers to <br />provide basic in-home caregiving and companionship to <br />seniors upon request. <br />Action Step #3. Work with City Council to identify and <br />allocate local, state, and federal resources to support the <br />expansion of no or low-cost caregiving services. <br />Action Step #4. Collaborate with a variety of community <br />partners, from social services agencies to technology <br />education programs, to offer respite resources and <br />information referral to help seniors and caregivers to <br />navigate healthcare systems and to purchase and set up <br />the necessary equipment and software to use telehealth <br />services. <br />City of San Leandro | Age-Friendly Action Plan
