<br />EXHIBIT A <br /> <br />SCOPE OF SERVICES <br /> <br />I. Obiective: The Consultant will perform consultant services itemized below relating to the City of <br />San Leandro's Safety Program: <br /> <br />A. Update existing environmental health and safety programs or develop required written <br />programs & policies as needed. 21 hours a month <br /> <br />B. Respond to management and employee questions and safety suggestions, voice mails, <br />em ails, special meetings with management as requested, regulatory agency issues, new- <br />hire safety orientation, recordkeeping, training coordination, providing updates to the plan <br />of action and training matrix, assist in the preparation of agendas for regular safety <br />committee meetings, handling safety committee issues, etc. 8 hours a month <br /> <br />C. Participate in the monthly safety committee meetings & coordinate issues that arise. <br />Included in office hours (8) <br /> <br />D. Prepare calendar of required training classes scheduled throughout the year, confirm the <br />instructor(s), rooms, equipment, etc. Included in office hours (B) <br /> <br />E. Conduct and document health and safety inspections of all employee areas of the City. <br />Inspection frequency for each area will be recommended to the safety committee after the <br />first round of inspections. The inspection will include a risk assessment code numbering <br />system. 5 hours a month <br /> <br />F. Conduct on-site employee safety training and refresher training monthly, including <br />employee environmental health & safety training requirements, new-hire safety orientation, <br />safety committee training, management training, providing maintaining the training material <br />binders, certificates, etc. 20 hours a month <br /> <br />G. Conduct ergonomic workstation evaluations of employees, on an as-needed basisand as <br />referred by Human Resources, for employees who experience discomfort at their <br />workstation or while at work, or train supervisors on ergonomic equipment set-up. <br />3 hours a month <br /> <br />H. Meet with affected staff and prepare reports on special issues that arise in the area of <br />Industrial Hygiene (e.g. air, soil, water, lead, asbestos samples, and reporting on the safe <br />practices for employees to work with/in these environments). As needed / hourly rate <br /> <br />I. Provide on-site medical surveillance, on an as-needed basis, to employees in compliance <br />with annual hearing testing, and respiratory protection medical evaluations. <br />Onwsite hearing tests (Van) charged at $20. Per person. <br />On-site respiratory medical evaluations (Van) charged at $70.
