<br />-- <br /> <br />Alameda Countywide Homeless and Special Needs HousingP/an <br /> <br />2006-2007 ACTION STEPS: MEASURING SUCCESS AND REPORTING OUTCOMES <br /> <br />In order to identify successful approaches and target resources toward best practices, it will <br />be essential to track consumer and program data and regularly analyze outcomes. Outcomes <br />should focus on real change in people's lives, and be meaningful, measurable, and <br />realistically within the capacity of both the providers and consumers to achieve. In addition, <br />systems and programs should be regularly assessed through collecting and analyzing data <br />that measures effectiveness and efficiency in achieving stated outcomes. <br /> <br />Strategy M-l recommends the coordination of data tracking and outcome measurement <br />between systems. Because each system is in the process of upgrading or adding new <br />functionality in its data collection system, now is the time to coordinate data collection and <br />reporting and address the many practical and ethical considerations, as well as legal <br />restrictions, regarding how confidential information is recorded and shared. Coordination <br />will need to be phased in. <br /> <br />2006~2007 action steps for key management-level liaisons from each major system <br />include: <br /> <br />> Developing a comprehensive understanding of data collection systems, reporting <br />requirements, and ability to report on the same client-level data in the same way. <br /> <br />> Collaborating to develop memoranda of understanding between systems that outline <br />work plans and time lines for establishing common data definitions and reporting <br />functions. <br /> <br />> Establishing baseline data from which future changes will be measured. <br /> <br />> Identifying potential outcomes that are meaningful and measurable, reasonably within <br />the systems' ability to influence them, and consistent across systems. <br /> <br />2006-2007 Action Plan-Page 9 <br />
