Gap Grant Program Application
Cycle #3 (06/07 - 07/08)

PART 4: DETAILED PROJECT INFORMATION

A. Project Description: Provide a detailed yet concise project description. Include
existing conditions and all relevant project history. Describe specifically what
the proposed grant funds would be used for including specific deliverables.
Please limit to 300 words or less.

In response to one of the most frequently noted needs identified in ACTIA's 2003
Gap Needs Assessment (i.e., the need for "Same-Day" paratransit services), the
cities of Hayward and San Leandro are submitting separate, but coordinated,
applications for Measure B Gap funds to support what together represents a pilot
paratransit service designed to enable over 2,000 older adults and persons with
disabilities who live in Central Alameda County to access subsidized same-day
paratransit services. Two versions of same-day service will be offered:
A) A "Demand-Response" same-day service where a passenger will receive a
ride within 45 minutes of their trip request; and
B) A "Reservation-based" Same-Day Service where a passenger may call at
one time of the day to reserve a ride later that same day.

Passengers will pay only one voucher (provided at no cost), plus $2, for each one-
way trip up to 10 miles in length. Historical data indicates that most trips in Central
Alameda County average 8 miles.

The City of San Leandro currently provides a form of same-day service through its
Shuttle paratransit program, however, passengers can only travel within the San
Leandro's City limits in this program. The Hayward Paratransit Program's current
Same-Day service is limited to medical trips only. The new service being proposed
will serve participants that are enrolled in both cities' Paratransit Programs, will
provide trips thoughout Central Alameda County, and will have no trip purpose
limitations. Hayward and San Leandro will contract seperately with a common
transportation service provider that will utilize sedans and lift-equipped vans to
deliver this service in a coordinated fashion.

Grant funds will be used to cover the following expenses:
(Hayward Only)*
For Years 1 & 2
- Program Administration (incld. eval & reporting) ....................$ 5,000
- Customer Outreach/Education (incld. printing) ...................... 2,500
- Trip Subsidies (for local same-day trips up to 10 miles)............ 42,500

- Sub-Total Year 1...........................................................................$ 50,000
- Su b- T otal Year 2......... .................. .................... ........ ................ ....$ 50,000

- TOTAL REQUEST {for 2-Year Grant Period).......................... ......$ 100,000

G:\Oliver\Paratransit\FY 2006-07\Gap Application - San Leandro Ride Today.doc
