<br />EXHIBIT A <br /> <br />SCOPE OF WORK <br /> <br />Term of Contract: <br /> <br />July 1, 2006- June 30, 2007 <br /> <br />Agency: <br /> <br />Davis Street Family Resource Center <br /> <br />Address: <br /> <br />3081 Teagarden Street <br />San Leandro, CA 94577 <br /> <br />Contact Person: <br /> <br />Eowyn Gorman, Client Services Director <br /> <br />Phone: <br /> <br />510~347-4620 <br /> <br />Project Name: <br /> <br />Comprehensive Family Support Services <br /> <br />PROJECT GOALS AND OBJECTIVES <br /> <br />The Family Support Services Program will provide food, clothing, PG&E energy assistance vouchers, motel <br />vouchers, job attainment and retention services, and linkages to long~term assistance such as childcare and <br />health services for low~income, families, individuals and seniors of San Leandro, reducing the risk of <br />compromising their self-sufficiency. <br /> <br />MAJOR ACTIVITIES <br /> <br />Families will receive a week's worth of groceries (canned goods, bread, produce, dairy and meat products), <br />and other services as needed, including: clothing/household items, emergency motel vouchers, PG&E utility <br />assistance, nutrition education and life skills training designed to teach the participants how to budget and <br />shop with limited resources and fixed incomes, information and referral to services not provided (such as rental <br />assistance, shelter, etc), job preparation and placement'services, internal linkages to counseling and childcare, <br />free acute medical care, and enrollment and referrals to affordable health care plans. Referrals are made to <br />other agencies for specialized services such as housing and additional food resources. <br /> <br />PERFORMANCE MEASURES <br /> <br />SUBRECIPIENT SHALL, FOR THE TERM OF THE CONTRACT use evaluation tools that consist of a client <br />intake interview, follow-up with subsequent visit(s) and re~certification, and documentation of frequency of visits <br />and intake notes. To track this grant's outcomes, Subrecipient will monitor client's housing and employment <br />status. The performance measures are: <br /> <br />1. Five hundred (500) families will be served each month, totaling 6,000 families served within twelve (12) <br />months. Case management services are provided to assess each family's needs, and the above~ <br />mentioned services are provided accordingly. <br /> <br />14 <br /> <br />COSO Agreement with Davis 51.
