<br />Amendment to the Massage Ordinance <br /> <br />Page 2 of2 <br /> <br />The application process requires submittal of detail information regarding location, individual <br />background, information regarding the specifics services to be provided, a livescanJbackground <br />check as well as the following before a permit is approved: <br /> <br />. Two passport size photo's <br />. Drivers license or identification card <br />. Acupuncture and <br />Acupressure establishments and their employee's conducting these services will be subjected to <br />the same permit review and approval process as a typical massage establishment prior to their <br />conduct of business. <br />
