EXHIBIT A <br />SCOPE OF SERVICES <br />Background <br />A roof survey was conducted in 2006 by Skyline Engineering, Inc to survey the condition of all <br />roofs on City owned buildings. Several roofs where noted in poor condition and appropriate <br />repair and replacement was recommended. This is the Phase II of a three phase program to <br />repair and replace deficient roofs. <br />This contract is to provide professional services to the City for design, bidding and construction <br />support for the Roof Repair and Replacement Project '08 - `09. The scope for this project <br />includes the repair of four (4) roofs on buildings owned by the City: <br />1) Main Library -Karp Room (back/rear built-up rock roof). <br />2) Fire Station #12 -built-up roof area. <br />3) Public Works Shipping/Receiving and built-up area ofgarage -including new <br />skylights to replace existing skylights. <br />4) South Offices mechanical roof area -including spec section for duct removal and <br />reinstallation. <br />Scope <br />1. Asbestos Testing (Per proposal from Skyline Engineering, Inc.). <br />2. Two (2) design field inspections of the site in order to verify construction and conditions. <br />3. Review plans of existing buildings supplied by the City if available. <br />4. Discuss the findings of the field inspection and provide recommendations on repair and <br />replacement. <br />5. Develop design repair/replacement drawings of the roofs. Provide a specification section <br />for the removal and reinstallation of HVAC duct work for the South Offices. <br />6. Prepare construction specifications in City's standard format. <br />7. Calculate quantities and prepare cost estimate. <br />8. Obtain building permit from City's Building Department. <br />9. Construction support services <br />a. Assistance through bidding, preparation of addenda, attending pre-bid conference. <br />b. Interpret construction documents, review shop drawings and other contractor <br />submittals, respond to RFI's and change order requests. <br />c. Review City's front end documents of the spec package. <br />d. Observe construction operations and site conditions. <br />e. Provide inspection services as necessary with formal written reports after each <br />visit; deliver to the City and contractor. <br />f.
