goals and performance standards stated in the Scope of Work. Substandard performance as determined by <br />the City will constitute noncompliance with this Agreement. Contract suspension or termination procedures <br />will be initiated if action to correct such substandard performance is not taken by the Participant within a <br />reasonable period of time after notification by the City. <br />Participant shall submit to the City written quarterly and narrative reports which shall be due on the fifteenth <br />(15~) day of the month immediately following the report quarter. The quarterly and narrative reports shall <br />include, but not be limited to, the following data elements: <br />A. Title of program or project and description of goals and activities, <br />B. Service areas (i.e., citywide, applicable census tracts, etc.), <br />C. Beneficiaries' Data: See attached Quarterly Performance and Narrative Reports, Exhibit E. <br />D.
