San Leandro Community Assistance Funding <br />GRANT APPLICATION <br />ORGANIZATION: San Leandro Public Library Foundation/Project Literacy <br />PROGRAM TITLE: Project Literacy <br />2. Objectives -Program Outputs and Outcomes <br />Please submit no more than two Objectives. In many cases, only one Objective may be sufficient. See Page 14 for Example of <br />Completed Program Objectives Form. <br />Objective #1 <br />a. Program Premise (if..., then...): <br />If individuals with low literacy skills (8'h grade or below) participate in an adult literacy program, then the probability <br />will increase that the individual will gain literacy skills and apply those skills in situations of family, job, education, social or <br />community life and personal development, thus increasing their independence and self-sufficiency. <br />b. Program Activities (Describe the services to be provided that will be in your agency's Scope of Work): <br />One on one tutoring, small group sessions, Computer Lab, Families for Literacy; affordable: free; accessible: <br />convenience-based schedule/location; client customized: learner-centered curriculum. <br />c. Program Outputs (The direct quantifiable products of program activities, such as the number of meals provided or <br />participants served. Program outputs should be measurable and realistic): <br />BY JUNE 30, 2009.(12 MONTHS): <br />200 students served (annual minimum of 19,200 volunteer tutor service hours) <br />BY JUNE 30, 2010 (24 MONTHS): <br />250 students served (annual minimum of 23,200 volunteer tutor service hours) <br />d. Program Outcomes (Program Outcomes =The benefit for program participants. Who benefits, what will change, by when, how <br />will you know.): <br />BY JUNE 30, 2009.(12 MONTHS): <br />Sixty percent of students with low literacy skills, who maintain their tutor and consistent attendance, will increase their independence <br />-and self-sufficiency as indicated by their demonstration of new applications o literacy skills within the context of work, family, community <br />or personal life. <br />BY JUNE 30, 2010 (24 MONTHS): <br />Seventy five percent of students who enter the adult literacy program in the year 2008-2010 and who maintain their tutor and consistent <br />attendance will increase their independence and sufficiency indicated by their demonstration of new applications of literacy skills within <br />the context of work, family, community, and personal life. <br />e. What are your sources of data? (List only those sources to which you have access): <br />Client intake/interview records Learner Gains survey <br />Clients Rosenthal Reading Assessment <br />Professional staff CAL PEP (State Library Literacy) six month questionnaire <br />Tutors Families for Literacy parent survey (pre -post) <br />Monthly Tutor Reports State Library Roles and Goals Evaluation <br />Observations provided by client's personal contacts (within network of work, family and community life) <br />f. What data collection/methods will you use? (If you have not tested a method, state when you will do so): <br />Monthly submission: Tutor reports (hours, goals, accomplishments) <br />Quarterly submission: Learner Gains survey, staff observation, tutor/learner interview surveys <br />Six month submission: CAL PEP questionnaire <br />Annual submission: Client intake and interview, update record as needed; Families for Literacy <br />Parent pre/post survey; Re-assessment test, California State Library Roles and Goals Evaluation <br />
