2. Thirty-one thousand five hundred (31,500) meals will be provided per month, totaling 378,000 within <br />(12) months. <br />3. Forty percent (40%), or 200 families, who accessed the basic needs services will demonstrate <br />increased financial or housing stability as indicated on their intake assessment and follow up at the 4 <br />and 12 month reassessment periods. Families will become more self-sufficient by maintaining their <br />housing for at least three months following use of theses services. <br />4. Forty percent (40%), or 200 families, will report a decrease in the risk of compromising their self- <br />sufficiency by maintaining employment during the first month following access to our services. <br />5. Forty percent (40%), or 200 under-employed families who receive comprehensive support services, will <br />reduce the risk of losing their jobs by maintaining employment during the first month following their <br />crisis or use of services. <br />TIMELINES <br />By October, 1, 2008, Davis Street will have accomplished: <br />• Provide 1500 families with basic needs services to include any of the above mentioned services <br />including case management to monitor and track progress. <br />• Distribute approximately 94,500 meals, the equivalent of one weeks worth of groceries to approximately <br />1500 families <br />By January 1, 2009, Davis Street will have accomplished: <br />• Provide 3000 families with basic needs services to include any of the above mentioned services <br />including case management to monitor and track progress. <br />• Distribute approximately 189,000 meals, the equivalent of one weeks worth of groceries to <br />approximately 3000 families <br />By April 1, 2009, Davis Street will have accomplished: <br />• Provide 4500 families with basic needs services to include any of the above mentioned services <br />including case management to monitor and track progress. <br />• Distribute approximately 283,000 meals, the equivalent of one weeks worth of groceries to <br />approximately 4500 families <br />By July 1, 2009, Davis Street will have accomplished: <br />• Provide 6000 families with basic needs services to include any of the above mentioned services <br />including case management to monitor and track progress. <br />• Distribute approximately 378,000 meals, the equivalent of one weeks worth of groceries to <br />approximately 6000 families <br />• Approximately 200 families, who accessed the basic needs services will demonstrate increased <br />financial or housing stability as indicated on their intake assessment and follow up at the 4 and 12 <br />month reassessment periods.
