Alameda County Transportation Improvement Authority <br />Special Transportation for Seniors and People with Disabilities <br /> percent of San Leandro residents are <br /> Kaiser members and access to Kaiser <br /> facilities is critical. Many residents also <br /> use Eden Medical Center in Castro <br /> Valley and a variety of other medical <br /> services outside the City limits. <br /> Please note that 100% of the MSL grant <br /> funds are used for transportation costs. <br /> There are no administrative costs charge <br /> to this service. <br />Interim Service We currently provide Interim Service. $0 <br /> The majority of the Interim Service trips <br /> are for medical trips outside the city <br /> limits and are funded with the MSL <br /> Out-of--Town Medical Transportation <br /> grant. If we do not receive an MSL <br /> grant for Out-of--Town Medical <br /> transportation, we will not be able to <br /> meet this need. We are not requesting <br /> additional grant funds for this service, <br /> but are instead noting that Interim <br /> Service is dependent on the MSL Out- <br /> of-Town Medical Trans ortation ant. <br /> <br /> $ <br /> <br />Total $7 5,000 <br />Additional Questions to determine minimum service levels gap errant funding: <br />1. Please explain any community-specific issues that have impacted your ability to not <br />meet minimum service levels? <br />The Flex Shuttle Service was designed based on extensive public input gathered <br />through a master planning process and continuing public outreach such as workshops <br />and surveys. Out-of--Town Medical Trips was added in 2005 based on input gathered <br />through surveys, workshops and focus group meetings which identified the need for <br />riders to travel outside the City limits to meet their medical needs. <br />2. Have you explored and documented other transportation options for seniors and <br />people with disabilities provided by non-profit organizations in your community that <br />might also close this service gap? Please describe. <br />Staff routinely assists residents with their transportation needs, providing information <br />about the variety of transportation services available. Transportation options include: <br />family, friends, taxi, AC Transit/Bart, East Bay Paratransit, residential facilities that <br />provide transportation and hospitals that offer shuttle services. Local non-profit <br />resources are very limited. Alzheimer's Services of the East Bay offers transportation <br />to clients, as well as, Stepping Stones an organziation that works with the <br />C:\Documents and Settings\kmyers\L,ocal Settings\Temporary Internet FileslContent.Outlook\OES 1NYN2\MB Annual Program <br />Application Cover FY0910.doc <br />Page 15 of 18 <br />
