Our "one-stop shop" Family Resource Center maximizes <br />dwindling resources by providing supported housing services to low income families in need of <br />safe and affordable housing. At the same time, DSFRC and the City of San Leandro will work <br />together to achieve the affordable housing goals. <br />DSFRC will incorporate a safe and affordable housing assessment at intake; will advocate for <br />safe and affordable housing for San Leandro residents and DSFRC clients; will provide referrals <br />for housing and assist with the application process and finally will deliver comprehensive <br />supportive services to at-risk and low income families. <br />The case management follows a holistic model that includes our family advocate, school <br />personnel, a mental health clinician an employment coordinator and housing specialist. <br />The Youth Anti-Violence Program will be expanded to incorporate "supported housing" needs of <br />low-income families, meeting the affordable housing needs of those with the most risk and least <br />access. <br />Consulting Services Agreement between June 15, 2005 <br />City of San Leandro and Davis Street Family Resource Center Page 16 of 18 <br />
