c. Dependent Care Flexible Spending Account with a maximum employee <br />pretax contribution of five thousand dollars ($5,000.00) per benefit plan year. <br />The City will maintain a "core flex" medical benefit plan. The core shall consist of <br />the PERS Medical Plan with adesignated co-payment and the existing dental plan. <br />Under CaIPERS rules, the City will contribute sixty-four dollars and sixty cents <br />($64.60) per employee per month towards the health plan. <br />The City will contribute the following monthly amount, including the PERS Medical <br />Plan "employer minimum share" contribution, toward the election of medical <br />benefits in the plan <br />Employee $ 389.38 <br />Employee +1 $ 778.76 <br />Family $1,012.39 <br />Each January, these amounts will be adjusted by an amount equal to the change <br />(increase or decrease) in the Kaiser Bay Area/Sacramento (BA/S) Premium. <br />Employees electing to enroll in the PERS Medical Plans will be required to <br />contribute the difference between the above amounts and the actual cost of the <br />medical plan premium. Such contribution shall be on a pre-tax basis. <br />Employees electing not to enroll in the core medical plan (non enrollment in the <br />medical and dental plan) and who demonstrate to the satisfaction of the City <br />enrollment in another medical plan, shall receive one hundred and fifty ($150.00) per <br />month for their non-election of medical/dental benefits. In the event both spouses <br />are employed by the City and eligible to enroll in the City's medical benefits plan, <br />one employee may elect not to enroll in the medical plan and will receive the one <br />hundred and fifty ($150.00) per month payment, if enrolled under the spouse's <br />coverage. <br />Employees electing not to enroll in the medical plan, but continuing enrollment in <br />the dental plan, shall receive one hundred dollars ($100.00) per month for their non- <br />enrollment. <br />Re-enrollment in the medical and/or dental plan shall be allowed only based upon a <br />qualifying event as defined by the IRS codes or during an open enrollment period. <br />23.2 Under Ca1PERS rules, the City will directly contribute the "employer minimum <br />share" of sixty-four dollars and sixty cents ($64.60) towards retiree health coverage. <br />Additional retiree health contributions will be made on a reimbursement basis as set <br />forth below. The amounts listed below are inclusive of the PERS Medical Plan <br />"employer minimum share" contribution. <br />23.3 For employees retiring prior to March 1, 1998, the City shall pay the contributions <br />required by the health plans' two-party rate for retired City employees who were <br />assigned to classifications represented by the San Leandro Police Officers' <br />Association and who are currently members of one of the City's Ca1PERS health <br />]7 <br />
