5. Planned Action to Develop an Institutional Structure <br />Public agencies, for-profit and non-profit private organizations, all play a part in the <br />provision of affordable housing, social services, neighborhood revitalization and <br />economic development. The City's Housing Services Division has the primary <br />responsibility for implementation of the Consolidated Plan and the Housing Element. <br />The Division works closely with the Redevelopment Agency (Office of Business <br />Development) in the implementation of housing projects and programs. Non-profit <br />agencies will also provide social services to the community this year. <br />The Alameda County Housing and Community Development (HCD) Department is the <br />lead agency in the implementation of the HOME Consortium's Consolidated Plan. HCD <br />also administers the HUD homeless programs and the Mortgage Credit Certificate <br />(MCC) Program for home ownership. <br />6. Planned Action to Enhance Coordination <br />The City will utilize non-profit agencies to assist in implementing the 2004-05 programs. <br />The City participates in the HOME Consortium composed of Alameda County and the <br />cities of San Leandro, Hayward, Fremont, Alameda, Pleasanton, Livermore, and Union <br />City. As a member of the County-administered MCC program, the City receives <br />certificates through the same. The Alameda County Housing Authority and the City <br />have an agreement that the Housing Authority can operate the Section 8 Program <br />within San Leandro. San Leandro council member, Surlene Grant is the new chair of <br />the Housing Authority. The City is a member in the Continuum of Care Council, a <br />coalition of homeless service providers and local jurisdictions, that responds to the <br />homeless needs in the County and develops a joint application to HUD Super NOFA for <br />McKinney related homeless grants. The City also partners with private sector mortgage <br />lenders to implement the San Leandro First Time Homebuyers Program. <br />7. OMB Performance Measures <br />CPD Notice 03-09, Development of State and Local Performance Measurement <br />Systems for Community Planning and Development Formula Grant Programs require <br />the jurisdiction to develop a measurement system. The City of San Leandro views the <br />Five Year Consolidated Plan and component Annual Action Plans as the performance <br />measurement system for all housing and community development programs within the <br />City. The Consolidated Plan identifies needs and problems within the city and contains <br />a strategy with solutions, addressing the needs and problems. All available resources <br />(inputs) are identified and used by the activities to achieve their desired outcomes. The <br />Plans contain long term and annual outputs and outcomes. Exhibit A in the <br />Consolidated Action Plan provides a summary of the desired outcomes, for example, <br />"preserve existing housing for low and moderate income households". The output for <br />the outcome would be to rehabilitate nine single family homes in this program year and <br />to complete sixteen units over a five year period. <br />Action Plan FY2004-OS <br />City of San Leandro <br />Page 29 <br />
