EXHIBIT A <br />SCOPE OF SERVICES <br />General <br />This scope of work is in additional to the current existing contract between the City of San <br />Leandro and John Cahalan Landscape Architect. The contract will provide for John Cahalan <br />Landscape Architect to provide additional services in compliance with Caltrans additional <br />requirements and change in policy, which occurred between the date of the existing contract and <br />now, such as requirements for Fact Sheets for Exemptions, Storm Water Management Report, <br />Additional plans and specifications submittal and reviews to Caltrans, Metrication of plans and <br />specifications, preparation of specifications in Caltrans Standards and additional review meetings <br />with Caltrans. Other services to be provided by the consultant, which were not originally part of <br />the existing contract, but required by Caltrans are Right-of--Way Certifications and Traffic <br />Management Plan. <br />Purpose and Scope of Services <br />The purpose of this extra service is for the consultant to provide services to comply with Caltrans <br />requirements as necessary to obtain its approval of the PSR/PR and subsequently Caltrans <br />approval for the project. The scope of services include the following tasks: <br />1. Fact Sheets for Exemptions <br />Consultant shall provide Caltrans all necessary fact sheets required to grant exemptions to <br />construction items, such as the entryway monuments and other items requiring exemptions. <br />Consultant will also respond to all review comments from Caltrans during the processing of the <br />fact sheets. <br />2. Storm Water Management Report <br />Consultant shall provide the storm water management report required for the project in <br />compliance with Caltrans requirements and respond to all review comments associated with <br />providing this service required for the approval of the PSR/PR for the subject project. <br />3. Traffic Management Plan <br />Consultant shall provide the Traffic Management Plan for the project, in compliance with <br />Caltrans requirements and as may be required to obtain the PSR/PR approval. The plan will be <br />required to be included in PSR/PR package, as necessary, but especially will be required to <br />included in the plan set. The plan will include lane closure detours and charts, and traffic control <br />plans. Consultant shall be required to provide to respond to all review comments related to this <br />services. <br />4. Right-of-Way Certifications <br />Consultant shall prepare, submit and revise as necessary the Right-of--Way forms related to the <br />approval of the PSR/PR to Caltrans, in accordance with the Caltrans Right-of--Way Manual. <br />Doolittle Drive Entryway Improvements April 7, 2004 <br />Project No: 230-43-155 Page 15 of 18 <br />
