EXHIBIT A <br />SCOPE OF SERVICES <br />Project Description: <br />The City of San Leandro plans to replace two existing boilers and add one new boiler in the <br />basement of the City Hall building located at 835 East 14`h Street. <br />Scope of work: <br />Design includes minor reconfiguration or modifications of doors and walls, extending conduits, <br />replacing two existing boilers, and placing of one new boiler, one new expansion tank, and one <br />new heating pump. Provide architectural, mechanical, electrical and plumbing as required for <br />the relocation of bailer room. Provide architectural services for the future chiller room. <br />Consultant is responsible for producing construction documents that are complete, biddable, and <br />buildable. The responsibility is not discharged by issuance of documents for construction or <br />receipt of a building permit. All work shown shall conform with the standard plans and details. <br />The City of San Leandro has standard plans and details. <br />Design: <br />1. Visit the site and perform field inspection of existing conditions as required. <br />2. Review plans of existing building supplied by the City. <br />3. Attend meetings with City. <br />~. Review project for compliance with accessibility requirements and provide documents to <br />upgrade the facility's path of travel to the project area. <br />5. Assist in obtaining the building permit. <br />6. Assist in obtaining bids and in awarding the construction contract. <br />7. Prepare design specifications. <br />8. Prepare AutoCAD 2000 drawings. <br />9. Review of City's front-end documents. <br />10. Prepare engineer's cost estimate of the construction cost with each submittal. <br />Construction: <br />1. Services shall be on a time and materials basis. <br />Schedule: <br />60% Construction Documents 4 weeks from Notice to Proceed <br />and cost estimate <br />90% Construction Documents 2 weeks from receipt of 60% <br />and cost estimate submittal review comments <br />Final Construction Documents 1 week from receipt of 90% <br />and cost estimate submittal review comments <br />City Hall HVAC Repairs May 20, 2004 <br />210-18-126 Page 15 of 18 <br />
