EXHIBIT B COMPENSATION SCHEDULE AND HOURLY FEES Consultant will take over August 1, 2004 as the Broker of Record for ancillary consulting services (Life, AD&D, STD, LTD, EAP, Vision and Dental coverages) and will receive the existing commission that is currently being paid to the previous brokers/administrators. Consultant will request quotes and complete an analysis of the market with the intention to reduce costs on all ancillary coverages while maintaining or improving benefits. Consultant will provide quotes from our proprietary Programs as well as the open market for your review. Driver Alliant Programs Consultant will provide quotes from their programs that will lower costs while matching or increasing benefits. Driver Alliant Program commissions are as follows: Life & AD&D 2 - 5% Short Term Disability 2 - 5% Long Term Disability 2 - 5% EAP 5% Dental 5% Vision 5% If the City of San Leandro selects a Driver Alliant Program, the above stated commission would apply. Open Market Quotes Consultant will also provide quotes from the open market. These programs will allow the City of San Leandro to have a complete analysis of the market on all ancillary coverages. The industry standard commission levels for these lines of coverage are as follows: Life & AD&D 10% Short Term Disability 10% Long Term Disability 10% EAP 10% Dental 10% Vision 10% Not to Exceed Consultant agrees not to exceed commission amounts greater than $43,000 on all ancillary coverages. If the combinations of commission amounts are greater than $43,000, Consultant will remit that amount back to the City of San Leandro. If the combination of commission on all ancillary coverages adds up to a total less than $43,000, Consultant will NOT balance bill the City of San Leandro.
