EXHIBIT B <br />COMPENSATION SCHEDULE AND HOURLY FEES <br />Consultant will take over August 1, 2004 as the Broker of Record for ancillary consulting <br />services (Life, AD&D, STD, LTD, EAP, Vision and Dental coverages) and will receive the <br />existing commission that is currently being paid to the previous brokers/administrators. <br />Consultant will request quotes and complete an analysis of the market with the intention to <br />reduce costs on all ancillary coverages while maintaining or improving benefits. Consultant will <br />provide quotes from our proprietary Programs as well as the open market for your review. <br />Driver Alliant Programs <br />Consultant will provide quotes from their programs that will lower costs while matching or <br />increasing benefits. Driver Alliant Program commissions are as follows: <br />Life & AD&D 2 - 5% <br />Short Term Disability 2 - 5% <br />Long Term Disability 2 - 5% <br />EAP 5% <br />Dental 5% <br />Vision 5% <br />If the City of San Leandro selects a Driver Alliant Program, the above stated commission would <br />apply. <br />Open Market Quotes <br />Consultant will also provide quotes from the open market. These programs will allow the City <br />of San Leandro to have a complete analysis of the market on all ancillary coverages. The <br />industry standard commission levels for these lines of coverage are as follows: <br />Life & AD&D 10% <br />Short Term Disability 10% <br />Long Term Disability 10% <br />EAP 10% <br />Dental 10% <br />Vision 10% <br />Not to Exceed <br />Consultant agrees not to exceed commission amounts greater than $43,000 on all ancillary <br />coverages. If the combinations of commission amounts are greater than $43,000, Consultant will <br />remit that amount back to the City of San Leandro. If the combination of commission on all <br />ancillary coverages adds up to a total less than $43,000, Consultant will NOT balance bill the <br />City of San Leandro. <br />Consulting Services Agreement between July 7, 2004 <br />City of San Leandro and Driver Alliant Insurance Services Page 16 of 17 <br />
