a. Subgrantee shall screen San Leandro residents to determine eligibility for the <br />Rental Assistance Program (RAP) and the Critical Family NeedslHousing Assistance <br />Programs (CFN/HA). When appropriate, Subgrantee shall prepare and approve <br />applications for RAP and CFN/HA, securing documentation, references and written <br />verifications. Subgrantee shall include the number of San Leandro households served <br />and the status of RAP loan guarantee repayments in the quarterly reports. <br />5. Subgrantee shall request and keep records of client information and action taken <br />or referrals given. Wherever possible client information shall include names, <br />addresses, telephone numbers, household size, race, ethnicity and the sex and age of <br />the Head of Household. If client refuses to provide information, it shall be noted and <br />counselors shall use their best judgment in completing intake information. A copy of the <br />intake form shall be provided to the City with the first quarterly report. <br />6. Subgrantee shall assist San Leandro Residents with short-term financial aid by <br />offering a security deposit guarantee and in some cases a guarantee for outstanding <br />rent due to a temporary financial setback. <br />7. Mediation. Subrecipient will contact parties in tenant-landlord dispute (when <br />there appears to be an opportunity for resolution), and negotiate a resolution. <br />8. Submit reimbursement claims using the City's invoice form or comparable form <br />at least quarterly with documentation of claim including copies of time sheets, payroll <br />stubs, DE3 or employment or contract letter for personnel costs, invoices or billings. <br />19 <br />
