EXHIBIT A <br />HPRP CONTRACTOR'S SCOPE OF SERVICES <br />FOR SAN LEANDRO HOtiSING RESOURCE CENTER <br />Services to be Provided <br />The Contractor will provide an array of services to identify and assess a household's housing crisis, and <br />provide appropriate financial assistance, housing location and stabilization services and broker access to <br />other critical services, particularly workforce services, with other programs and agencies to address the <br />housing crisis. Contractor will provide the following services: <br />1. Outreach, En~a~ement and Referral <br />The Contractor will receive referrals from a variety of different sources including Eden Information and <br />Referral's "211" hotline which will serve as a centralized referral source to the program. <br />The Contractor must coordinate with 211 and receive appropriate referrals from 211. Everyone Horne <br />will manage an outreach and marketing campaign for the program and the Contractor will coordinate <br />with this effort. The Contractor may also publicize the services and availability of the program. <br />2. Risk Assessment and Housing and Plan Development: <br />The Contractor will assess all clients referred through referral mechanisms established for the HPRP <br />program. Completion of this assessment will assist the Contractor in identifying the appropriate level of <br />financial assistance, if any, and the appropriate mix of supportive services. No household may receive <br />HPRP financial assistance unless they have been assessed by a case manager or other authorized <br />representative to determine the appropriate type of assistance <br />Contractor will be expected to utilize a countywide risk assessment and targeting tool to be designed <br />prior to program implementation. This tool will be used to assess potential clients for eligibility and to <br />help determine the appropriate level of service. This instrument is being designed with input from both <br />jurisdictional staff and service providers and may be modified locally to account for specific local <br />targeting priorities. This tool may be adjusted during the course of the contract period and selected <br />providers will be expected to implement any modifications made to the tool during the course of the <br />contract within the context of the program. <br />Providers will also be required to verify client household income, using the income verification <br />requirements and practices for the Section 8 Housing Choice Voucher Program, [See Federal Code of <br />Regulations 24 CFR Part 5.] <br />Once the client household has been assessed, the Contractor will work with the client to develop an <br />appropriate housing and income stability plan. The plan will identify steps that the household will take <br />to secure or stabilize their housing situation, ensure that household costs are covered with a combination <br />of income and services for which the household may be eligible (such as food, benefits, training and/or <br />employment services and subsidized child care), reduce the impact of utilities on the household's <br />Ex.
