EXHIBIT A <br />SCOPE OF WORK <br />Term of Contract: July 1, 2010 -June 30, 2011 <br />Agency: Girls Incorporated of Alameda County <br />Address: 13666 East 14th Street <br />San Leandro, CA 94706 <br />Contact Person: Rebecca Cannon, Chief Mental Health Officer <br />Phone: 510-357-5515 <br />Project Name: Pathways Counseling Center's Family Strengthening Program <br />PROJECT GOALS AND OBJECTIVES <br />The Pathways Counseling Center's Family Strengthening Program will provide individual and family therapy <br />services and Family Communication Workshops that enable individuals and families to address psychological <br />and communication problems within their families thus increasing the likelihood that these problems do not <br />escalate into parental abuse or adolescent acting-out behavior. The program will also provide family crisis <br />intervention services., in which children and families can engage in to develop communication skills and <br />psychological capacity to overcome the current "hot" crisis of their families and reunite as a family, and respite <br />care intervention for pre-delinquent and delinquent youth. Services will be delivered in Spanish and English. <br />MAJOR ACTIVITIES <br />Pathways services include: <br />1. Individual and family therapy services, and <br />2. Family Communication Workshops. <br />Mental Health Clinicians will conduct clinical assessments (measured on a 10-point scale conducted at: intake, <br />subsequent 90-day intervals, and termination). Participating family members will complete communication <br />skills assessments as well as survey evaluations regarding quality of treatment services. <br />The Family Strengthening Program, in conjunction with the Alameda County Crisis Receiving Home (Malabar <br />House), also provides: <br />1. Family crisis intervention, and <br />2. Respite care intervention for pre-delinquent and delinquent youth. <br />Mental Health Clinicians conduct family functioning matrices as well as child behavioral assessments. <br />Program participants complete survey evaluations regarding quality of crisis intervention and/or respite <br />intervention services. <br />-13- <br />CDBG Agreement with Girls Inc.
