7. Program Monitoring and Reporting <br />a) Contractor shall submit written quarterly reports to the City. The quarterly reports shall include, <br />but not be limited to, the following data elements: <br />i.) Title of program, listing of components, description of activities/operations. <br />ii.) Service areas (i.e., citywide, etc., including applicable census tracts). <br />iii.) Goals-The projected goals, indicated numerically, and also the goals achieved (for each <br />report period). In addition, identify by percentage and description, the progress achieved <br />towards meeting the specified goals; additionally identify any problems encountered in <br />meeting goals. <br />iv.) Beneficiaries- Provide the following: <br />1. Total number of direct beneficiaries. <br />2. Percent of total number of direct beneficiaries who are: <br />a) Low and moderate income <br />b) Low income <br />c) Black, not Hispanic origin <br />d) White, not Hispanic origin <br />d) Hispanic <br />e) American Indian/ Alaskan Native <br />fj Asian or Pacific Islander <br />g) Female Headed Households <br />v.) Other data as required by City. <br />b.
