CONDITIONS OF APPROVAL <br />PLN2009-00017 <br />642 Dolores Avenue <br />Assessors Parcel Number 077-0510-018-04 <br />Beth Sholom Preschool (Applicant) <br />San Leandro Hebrew Congregation (Property Owner) <br />I. COMPLIANCE WITH APPROVED PLANS <br />A. The applicant and/or property owner shall comply with Exhibits A through D, <br />attached to the staff report dated September 3, 2009, except as hereinafter modified. <br />(Exhibits are on file at the City of San Leandro Development Services Department, <br />835 East 14th Street, San Leandro, California). <br />Exhibit A -Cover Project Data Notes (Sheet AO) <br />Exhibit B -Architectural Site Plan (Sheet A1) <br />Exhibit C -Floor Plans (Sheet A2) <br />Exhibit D -Exterior Elevations (Sheet A4) <br />B. The applicant and/or property owner shall be responsible for assuring that any <br />successor in interest who assumes responsibility for the zoning approval is <br />informed of its terms and conditions. <br />II. USE RESTRICTIONS <br />A. This conditional use approval permits the expansion of the existing child care <br />facility serving up to 21 children to a total of 69 children, with nine staff members, at <br />the Beth Sholom Preschool, 642 Dolores Avenue, Assessor's Parcel Number 077- <br />0510-018-04. <br />B. Any change to the number of children or staff members shall be submitted in <br />writing for the review of the Community Development Director, who may approve <br />the request administratively or direct it to the Board of Zoning Adjustments for a <br />formal public hearing. <br />C. Permitted hours of operation are 7:30 a.m. to 6 p.m., Monday through Friday. <br />Children are allowed in the outdoor play area between the hours of 9 a.m. and 5 <br />p.m. <br />D. This use shall operate in conformance at all times with the State of California <br />childcare licensing requirements for License 9058 for a child care facility. <br />E. Parking for staff and parents shall be shall be designated at the existing temple <br />parking lot at the rear of the property, accessed from Juana Avenue. Drop off and <br />pick up of children shall be primarily directed to the designated drop off/pick up <br />area adjacent to the temple parking lot. <br />Conditions of Approval September 3, 2009 <br />PLN 2009-00017 Page 1 of 4 <br />
